چکیده

Abstract Introduction Cardiopulmonary Exercise Testing (CPET) and the Colorectal Physiology Operative Severity Score (CR-POSSUM) are increasingly used in colorectal surgical practice to risk-stratify patients preoperatively for planning level 2 HDU or ITU admission. This study aims generate a simple, objective pre-operative risk assessment model complement clinical judgement, determine objectively whom CPET is necessary pre-assessment. Method A retrospective cohort was conducted. Demographic perioperative data were gathered, CR-POSSUM score determined retrospectively. Mortality at 12 months considered primary outcome with readmissions up as secondary outcomes. Statistical analysis Binary Logistic Regression, odds ratios reported confidence intervals. new combined most significant individual predictors constructed. Results Some 167 elective included final analysis. Half of had planned admissions. The all-cause mortality 90 days 3.5% 11%. Readmission rate 30 10%, further 11.3% postoperatively. ASA, Scores correlated rates (p < 0.05). Current diagnostic performance using vs. identified 48% 35% ‘high risk’ 83.3% 56.5% vs 94.4% 76.1% sensitivity specificity, respectively. draws its predictive power mainly from CR-POSSUM. Conclusions centre produces outcomes above national average. process can be improved whilst focusing resources model.
